Underneath the different extension it is the same MPEG-4 container, so converting one you made yourself is close to a rename plus a clean rewrap, and it finishes in seconds with the picture untouched. There is one hard exception worth knowing before you start: films and shows bought or rented from the iTunes Store are also M4V, and those are locked with FairPlay DRM. How to convert M4V to MP4
Add the M4V, let the page rewrap it into an MP4 container, then save the result. Nothing is sent to a server, and for a file you created yourself the whole thing usually takes a few seconds because the video is copied rather than compressed again. If the M4V came from the iTunes Store it will not get past the first step, because FairPlay DRM stops any tool from reading it.

M4V and MP4 side by side
These two are closer than almost any other pair of video formats. Apple introduced .m4v to mark files meant for its own apps and store, but the box around the video is the same MPEG-4 container that .mp4 uses. Here is what actually differs once the file is converted.

One row this table cannot give you: DRM. If the M4V is an iTunes Store purchase or rental, none of the columns above apply, because the file never opens far enough for any of it to happen.
If they are the same container, what does converting achieve?
The extension is the whole problem, and it is a real one. Plenty of software decides what it will accept by looking at the file type before it looks at the video, so a perfectly ordinary H.264 file gets turned away for being called .m4v. Android players, web upload forms, older smart TVs and a fair number of editors all do this. Converting rewrites the container header rather than only the name, so what comes out is a plain MP4 the whole way through.
The honest tradeoff is that you gain compatibility and nothing else. The picture does not improve, the file does not get smaller, and if a player still refuses the MP4 afterwards then the container was never the issue. At that point it is either DRM or a codec inside the file that the player cannot decode.
Where the MP4 plays, and what stays out of reach
An H.264 MP4 with AAC audio is about as widely playable as video gets. That is what comes out of here whenever a re-encode is needed, and a rewrapped file keeps whatever it already had. The limits are worth stating just as plainly.
The output opens in browsers, on Android and Windows, in smart TVs and consoles, in editors like Premiere and Resolve, and in upload forms that reject .m4v on sight.
DRM is not a compatibility problem and converting does not touch it. A FairPlay protected M4V stays playable only in Apple's own apps, signed in to the account that bought it, and no tool anywhere changes that.
This page swaps the container and can pull out the audio. It does not trim, crop, rotate, change the aspect ratio or edit anything, so a video that is the wrong length or shape arrives at the other end the same length and shape.

The container is rewritten

If a stream is something MP4 will not carry as is, the file is re-encoded to H.264 and AAC instead. That path runs at roughly real time in a single threaded browser build, so a 90 minute file takes something like 90 minutes.

Why people run this conversion
Almost nobody converts M4V for quality reasons. The reasons are duller and more practical than that.
Something refused the file for its name
A course platform, a client portal, an Android phone or a TV app looked at .m4v and stopped there. The video inside was fine the entire time, which is why the fix takes seconds rather than an hour.
Renaming by hand is not quite the same job
Changing the extension in Finder works more often than not, because the bytes are already MPEG-4. It also fails in ways that are hard to diagnose, since the header still describes an Apple flavoured file. Rewrapping produces a file that is standard in its contents as well as its name, and it costs you a few seconds.
Nothing is compressed twice
Re-encoding a video always throws a little away, and doing it repeatedly across a library is how home movies quietly turn to mush. A copy of the stream avoids that completely, which matters most for the old footage that only exists in one place.
A mixed folder is not a problem
If a MOV or an AVI is sitting among your M4V files, add it anyway. It is converted along with the rest and labelled as whatever it actually turned out to be, rather than being rejected for arriving on the wrong page.
M4V usually means somebody's personal library
The files that arrive with this extension tend to be old iTunes era home videos, wedding footage a relative burned to a drive, kids' school plays, and camcorder tapes somebody spent a weekend digitising. Those are not files to feed through a stranger's server to fix a three letter problem.
